The cycle
Three answers to the same question, in one column each.
The self-rating, the manager's rating and the peer average land question by question, so the gap between how somebody sees themselves and how the team sees them is visible instead of argued about.

Their self-rating and whatever the visibility rules share with them, on their own page, without asking HR for a copy.
The manager sees their team
Scoped to the people who report to them rather than the whole company, the same way onboarding and leave decisions are scoped.
And some of it stays with HR
What the manager shares and what stays private is a setting, so a candid note does not become a surprise in the conversation.
